ORDERING/CONDITIONS AND POLICIES

| How to Order |
- Call 1-800-MED-LIST (633-5478), or 1-630-350-1717, or e-mail sales@mmslists.com, specify desired list(s), selections, materials, key code(s), postage class/automation discounts, services.
- Customer Copy Required - Please provide sample mailing piece, e-mail or fax copy, or telemarketing script to MMS-our fax number is 1-630-350-1896.
- Order Confirmation - MMS will email Order Acknowledgement summarizing list/service pricing and MMS’ General Terms and Conditions, which customer agrees to by signing and emailing to MMS.
- Order Production - Upon completion of (1)-(3) above, MMS will produce and ship customer order.

| Cancellations |
Customer list orders processed by MMS may be cancelled within 10 days of customer’s written approval of MMS Order Acknowledgement. Customer will be invoiced $25.00/M ($200.00 min.), plus List Service & Material Charges applicable. No cancellation of customer email orders after email broadcast date accepted; cancellations beforehand $495.00 flat.

| Commissions |
20% to recognized list brokers, 15% to recognized ad agencies on one-time list rental
only, unless otherwise noted. Inquire regarding special arrangements for 3rd party
unlimited use transactions. |

| Conditions of Use |
List use is subject to MMS’ General Terms and Conditions, including: subject to
owners’ advance approval; for a 1-time use unless otherwise agreed upon in writing.
Lists are seeded and are not provided on a net name basis unless otherwise agreed
upon in writing. Lists must be used within 90 days of shipping date or returned to
MMS or destroyed. If destroyed, affidavit to that effect must be provided to MMS.
MMS is not responsible for contingencies arising out of unauthorized use. List rental
price includes use of individual and/or organization name/address only; additional
charges will be incurred for use of other data. |

| Credit |
All orders, including third party contracts, require satisfactory credit references (three
suppliers with whom you currently have open accounts and customer bank) or advance
payment by check or credit card (MasterCard, VISA, or American Express). |

| Discounts |
Inquire for special prices available for high-volume or multiple uses over specified
period subject to list owners’ policies. |
|
| Pricing |
All prices and royalties are subject to change without notice. All written and/or verbal prices are subject to customer’s advance written approval of
MMS Acknowledgement of customer’s current order. |
|
| Rerun Policy |
Orders rerun at customer’s request after we have processed the initial order will be
invoiced at $15.00/M ($125.00 min.) plus any List Service and/or Material Charges. |

| Rush Orders |
Usually available for no extra charge. |
|
| Shipping |
Shipments are made via United Parcel Service 2nd Day Air at cost plus $50.00
handling unless otherwise requested. |
